Detailed TFS Rangers doc on how to migrate a TFS deployment (without upgrading)

TFS moves are one of the top issues Microsoft Support addresses. The MSDN docs provide good guidance for basic moves but there are lots of other factors companies may be dealing with such as domain changes and partial migrations that do not get captured.

I tried using the Integration Tools to migrate work items and source code from a TFS 2010 installation to a TFS 2012 installation a while back and it failed with a very cryptic error. I would like to see more guidance around how to handle specific errors based on Rangers' (or other users') experiences included in the guidance.
